My Endocrinologist Does Not Know About WPATH SOC 8 As Having Eunuch Mention
I saw my endocrinologist yesterday in the hopes of furthering my moving forward in my genderality. Im old and was actually scared with her mention of blood clots - possibly resulting from the use of estrogen. She understands that taking Testosterone is out of the question not on the table for discussion.
It appears that her organization still adheres to WPATH SOC 7 and when 8 came up in our discussion she asserted that the new upcoming 8 only is for addressing adolescents issues. She was unaware of it addressing Eunuchs at all. And her organization (a very well known one in NYC) has been telling me NO since I first went there for help.
Obviously Im concerned that my orchiectomy, although lifesaving, was just a huge concession to me and as well and at the same time another road block in my quest to be who i really am. And that, as best as i can express, is somewhere in the spectrum of male to eunuch to female and most certainly elsewhere beyond. I just want some peace in my life and in the name of safety i feel that those who are here to help with gender only offer me danger. i am feeling desperate and dont know which way to turn.

it is important to mention that i am seeking a penectomy which the urologist who performed the orchi said would be tricky at best to get approval for. further when i complained of continued soreness in the area, he suggested seeing a pain management doctor. that seems to be distancing himself from my care.
as it now seems to be not such a good idea for me to get estrogen therapy, i really believe that the further gender surgery is essential. ideally i’d love to have the prostate removed as well as the penis. both of these organs i feel doing stuff almost all the time and it drives me crazy. so it is these surgeries i feel are so essential to my well being yet impossible to achieve due to WPATH, insurance and the reluctance of the professionals who would do the work.

It's uncomfortable to read how tough/desperate things have been and are for you at the moment. I can only begin to imagine how dreadful it must be.
A key thing is that the WPATH SoC v.8 hasn't been formally adopted yet, it is just in final draft stage. So clinics very likely wouldn't be aware of it. They only tend to promulgate new guidance after it is in force. Still, their dismissive attitude towards WPATH is alarming -- it sounds like that person read a single magazine article of bulletin on one part of it. Less conscientious professionals tend to be like this, sticking to their old ways and just randomly browsing news to pick up on bits and bobs of development. Ideally they would be fully informed.
The suggestion to just stick with pain management is increasingly popular with health services because some bogus papers did the rounds claiming somatisation (all in the head) explains a lot of difficult cases and money can be saved by neglecting them rather than helping.
I'll be honest and say it does not sound like a great clinic. It also sounds like your options are limited... so when I say it sounds like they aren't very good I simply mean that to reassure you that you are not the problem, they are.

I was told in a thread elsewhere here on EA that any GOOD clinic should be aware of SOC 8 - it is not a secret!
The Doc I'm working with is definitely aware of it, and that while it's still a draft is at least somewhat using it (IMHO why I'm not having problems...)
It might be worth making a copy (especially of the Eunuch chapter, but there are lots of relevant / useful bits elsewhere in it) and giving it to your doc... Nothing like being able to quote chapter and verse w/ hard copy to back you up... Alternatively you could see if there are other centers you could work with, I'd imagine that as big as NYC is, there are probably several...

the others are not in network for insurance and this one is the biggest hospital in NY. The others
well i had a little fight because the orchiectomist had on their profile page that they accept my insurance but they dont. it appears that no one else is in network. id have to change plans and disrupt some other service im getting with another provider. not sure i want to do that either. but still figuring out what to do. and FYI, NYC is as open minded as a closed mind can be in many respects. particularly health care. oh, and also, i have considered that the endocrinologist did not want to let on how much she knew because then shed have to start with concessions
maybe? do you think that could be?

Patience! SOC 8 is almost here (about 2 more months). Then it will need to filter through the system. Good things come to those who wait, in this instance.

My understanding is that the blood clot risk from estrogen is primarily from orally administered estrogen, but not when taken sublingually, transcutaneously (patch or gel) or by injection.

I should have said transdermally rather than transcutaneously in my last post, but I guess they mean pretty much the same thing.
