Trouble sleeping and sitting!!
Hey guys, I'm looking for some info on chronic pain as well as orchiectomy and trt info.
So, ill start off with the chronic pain ive been having. Its been about 5 years now, and there are bilateral cysts above the testicles which my urologist is saying shouldn't cause pain. Even though thats right where I feel the most pain. He's sending me to see a physiotherapist because he's convinced its pelvic floor related...even though this is the second physiotherapist ive seen and ive done about a million kegels. I asked him if he would do orchiectomies but he said no and then I asked for a second opinion and im going to get that in sept.. My testicles also only hurt when the scrotum is hanging loose. From the research ive done this seems to be a rare thing. I take cold showers which helps, but only temporarily. Like I said in the title , they only hurt when im lying or sitting down which is pretty shitty as you can imagine. And fyi I dont have pain during sex or masturbation. Ive been looking into getting some boxers that come with a cooling fan and also a bed pad that keeps you cool at night. But honestly I dont like testicles, i see them as a weakness and gross so I wouldn't mind just getting rid of them for a smoother appearance down there and to be more comfortable for the rest of my life.
I have a pretty good idea of what life is like on trt. Ive heard that weekly injections are the best because bi-weekly seems to wear off during the second week. Bi-weekly would obviously be better though. The gel seems pretty easy too but its an everyday thing and isnt there also a smell and some greasiness? Im going to bank Sperm so as far as I know there shouldn't be any changes other than having to jab myself on a regular basis, right? Because from what ive learned you still get horny, have orgasms, erections, maintain muscle mass as long as you take testosterone.honestly it sounds kinda nice, im scared to go on a plane for 12 hours and be constantly reaching down and adjusting myself.

If your pain continues and becomes too debilitating. In the end they will give you orchidectomies. Given that you make a sperm deposit. You could live a perfectly healthy life on HRT. In Europe Nebido is available which is a 10-12 week injection that is very effective at maintaining optimum T levels. If you opted to have prosthetics, no one would know. You might prefer a smooth look. The darn things just get in the way. I think the main thing for you is to emphasise to the doctors the pain, discomfort and poor quality of life you have. They cannot ignore this. There is not an effective, non-addictive pain killer for testicular pain. There is really just one answer. If you went to a female urologist you might have a easier journey, I think male urologists just want to save testicles. As they wince!!

I dont hold myself out as an expert but I would give my own experience. When I was in my early 30s my libido started to decline. I also probably drank too much beer although that is speculation. I went to an endocrinologist who did all kinds of tests. In the end I was placed on testosterone. Well, it worked well but at that time I was going to his office for the injections. In time I was less attentive. Subsequently years later I tried the patches, gel, etc. They are to be charitable a royal pain. Eventually I discontinued altogether.
That was a long way around to get to the point. I was told that the testosterone would likely have an effect on my prostate. Im now in my 70s and yes that proved to be a major deal resulting in a TURP procedure about three years ago. It allowed the urine flow but not like when I was younger. Additionally it affected my ability to achieve erections; a problem which I did not previously have. The second undesired effect was retrograde ejaculation. I spoke to the urologist this past week and there is a new procedure coming that may eliminate those problems.
My strong laymans opinion is to get two or three opinions and get to the bottom of what is causing your pain. Even if you were to be castrated, you will still have a prostate as far as I know. It is not removed unless there is cancer involved.
My $0.02 for today.

I had a vasectomy that led to twenty years of uncontrollable chronic testicular pain.
I went to 5-6 different specialists. I had two procedures. Nothing helped until I found a urologist who performed my bilateral orchiectomy.
In retrospect it was the best thing I ever did.

As I understand it, you developed prostate cancer while on HRT. It would be normal protocol to remove it since it had been linked to prostate cancer. In my case, Ive never had a high PSA reading and no sign of prostate cancer although it evidently did lead to prostate hyperplasia and the difficulty with urination and subsequent TURP procedure.
My point in all this is if I had pain in my testicles (which I have never had) I would want a definitive diagnosis before castration. The bottom line is I doubt there is as high a chance for prostate cancer with natural testosterone than with HRT replacement. At my age I am not very concerned with prostate cancer. I regret the TURP procedure because of the impotence problem and retrograde ejaculation. But I had no choice based on current medical science. One has to urinate for sure.
My point in all this is if I had pain in my testicles (which I have never had) I would want a definitive diagnosis before castration. I had a vasectomy in 2014 and have since suffered recurring bilateral epydymylitis as well as bilateral cysts and now after a recent ultrasound bilateral hydroceles as well as intermittent prostate issues.masterbation and sex is painful. my dr has referred me to an urologist because he thinks I need surgery.. I was diagnosed with heart failure and A/F in 2015 and was put on a concoction of medication which pretty much devastated my sexual drive and testosterone levels.. I think my lowest blood test show my T levels at 7. I went through all the things like lost of strength,low libido, hot flushes etc etc. my T T levels slowly recovered and my last test it was at 19,5. But I’ve been left with very little libido, low energy levels and I’m basically impotent. I can still get semi erect but as soon as and sexual activity starts my balls start hurting and I loose what ever erection I had.. I’m at the stage now that I’m over my balls but I don’t want to go through the low T stuff again.. I’m going to ask for an orchi and a form of hormone replacement. I’ve read that Progesterone can be used to ward off a lot of the side affects of castration but it will not increase the sex drive and I’d be happy with that. I spoke I’ll just have to wait and see what happens. But I do empathise totally with you for reasons stated above.

Minor suggestion, there are a few different standards for T levels, so it is helpful if you can give the units... Assuming it is ng/dL then a level of 19.5 is still well below 'castrate level' (usually defined as under 50ng/dL)
Hormone Replacement can be a big help in handling most of the problems associated w/ castration, especially the mental health and bone density concerns... SOME folks report getting / maintaining sexual function w/ supplemental T, but it seems very unpredictable. An alternative that works on the issues, but won't do anything for your sex life is estradiol, (estrogen) which will probably result in some level of feminizing. There are also drugs called "SERMS" which supposedly act like estradiol, but don't have the feminizing issues.
If you have trouble w/ docs not wanting to do the orchie, you might want to talk w/ a Transgender center, they would likely be more sympathetic. If you care about appearances, a surgeon that is used to doing TG operations may do a nicer job.
Good luck in getting your needs handled.

Yeah I find the qualification of what is the normal level of testosterone very confusing.
