COVID travel restrictions and Dr Arnkoff
If I can't get a physician's or psychiatrist's letter supporting orchiectomy, does that mean a visit to Dr Arnkoff would be considered elective and therefore not "medical related" travel? Traveling from Canada that is. Are there any doctors like him in Canada?

You should give a little more information about what major city you are near. That would allow people familiar to advise you on travel restrictions.
Everyplace is so different. Some places have the honors system.
According to this page, the Detroit airport does not seem to list any restrictions aside from a mask.

The real issue is: what will be required at your home airport in Canada when you try to return?
A friend visited in from New York. The day after his return, someone from a health agency stopped by his apartment the next day to make sure he was in quarantine. I would not be surprised if they tracked his cell phone location during that week. Other cities have no restrictions.
For maximum protection, I suggest you purchase the an ENVO mask. www.envomask.com . They provide 100% filtration. It might take a month to receive. With a more contagious COVID, it's essential.
It's probably going to be most efficient to rent a car. If the office is still in Farmington Hills, the distance to the airport is 30 miles.
I had a lot swelling from one of my cords. I just wanted to go and lay down after. You probably will not have that. It's always possible.
How long do you plan to stay in Detroit after the procedure? It might be good to get the trip over with soon, and fly home in the afternoon.
Pain pills and driving will not work. Think through everything carefully.

You shouldn't have to cross the border. I don't know where you are, but in Ontario it's now a lot easier to get that sort of thing. I was able to get estrogen on informed consent.

I read today about a new policy that all international travelers must provide a COVID test before flying into the US starting on Jan 26.

I wouldn't read too much into anything in the US travel policies at the moment. With a new President, some of this may change in the next couple of weeks, either loosening some restrictions or creating more. If you're looking to come to the US, I'd hold off on making reservations for now, waiting for a couple of weeks for clarity as to what the new government will do related to travel.

I'm from Halifax, Nova Scotia.
You shouldn't have to cross the border. I don't know where you are, but in Ontario it's now a lot easier to get that sort of
I've been to a psychiatrist, a forensic psychologist, three doctors (five if we count walk-in clinics) and only one is willing to see me to prescribe anything more than a very, very low dose of cypro and/or spironolactone; she had misdirected me away from her practice and I only found her out when going to a pharmacy that offered HRT and learned that she did most of their prescriptions. I'm going to ask about orchiectomy, but I also know my luck well enough to want to have a backup plan.

I'm cisgender, but I'm taking HRT to suppress unwholesome sexual feelings related to a paraphilia. Given testosterone's role in the paraphilic sex drive, I'd rather like to be on a low dose of estradiol for cognitive and emotional health.

How are you feeling with the the drugs that you are taking?
It looks like you don't have very many flight options. Like most of them go through Toronto.

Ahhh, they want me to stop Zoloft for two weeks. I don't know if I could do that.

Pretty well, but I'm feeling like the additional benefit of the psych meds is wearing off. Increased HRT to compensate, wasn't taking enough for full T suppression.
