Went back today for the spermatic cord block. I hate needles and the doc walked in with a nice syringe in full view got my nerves and anxiety going. He spent time explain what he was going to do and what to expect or to experience. Got my nerves calmed down a lot. In short if this block worked for 6 hours in relieving my pain then it would time to do a denervation surgery leaving just an artery to supply blood to the right testicle. He is opposed to taking it out for hormone issues.

It was time for the shot and drop the pants for the shot. Weird having someone feeling around your junk to find the cord. Overall the shot was similar to a flu shot not painful at all. I was soon done and able to cover up my junk again and given instructions to call back on Monday with how the block performed and to do things that aggravated my pain issue. I returned home turned off my nerve stimulator and went to shoveling snow among other things that would leave me down and out for a day or two with pain. Strangely I’m closing in on 12 hrs since the block being done and haven’t experienced one not one horrible shot of pain or nausea from my chronic pain issue.

See what the coming days bring and hope it lasts even further for me since today was one of the least painful days I have seen in ages. I think we have finally found an answer to my pain situation

I know that the doctor was hoping the block would last for 6 hours but, that it lasted for 12 is not unusual.

About 2 years ago, I did three different tests of injections into the lumbar spine to see if a pain pump would help me with severe pain in my foot. The pain relief was magic. I went from 40 years of horrible pain to no pain at all. The goal was for the injection to work for 8 to 12 hours. Each one lasted far longer than that. One of the medications they tried lasted 72 hours. Unfortunately, the tests failed because I had some bad side effects.

Thanks and I’ve found help here and happy to return it others who may now or even down the road find themself in a similar situation as myself. Called the Dr office back and chatted with the nurse about my results. Oh yeah forgot the results.... dah!

The doc said a 6hr margin for relief but Friday came and went with me doing my hardest to bring on pain. Hell I was out shoveling the hell out of snow and felt good. Wow just Wow! The next day was a similitude story and mind you I have turned off my nerve stimulator and having no pain well beyond a 6hr window the doc gave me.

Sunday minor hints of a pain flare became evident and turned the device back on. Same for Monday and made the call

Back to the call nurse said she would pass along my much shorter result that I reported. The doc called me back shortly afterward. He gave me two choices, proceed with the cord denervation surgery under general which would be surgery number 11 in less than 12 years, or we can wait and see when or if the pain returns and do another block if I’m finding pain relief that lasts. I’m going for the later and see how long this block lasts and maybe knocked my pain flare down for a bit and go from there

Well another update. It’s been months since I had the block and given the choicest wait and see or request the surgery. I chose the wait and see and consult about this issue when I go back to the URO in May. Things are strangely still going well and the pain flare I experience earlier still has not return as of yet. The right ball still swells to some unbelievable size only to return to a normal size the next morning. Still up in the air what to do for surgery or not for the May appt. keeping the notebook updated so when I return in May I can tell the doc one way or another.

Been awhile since I have updated. The pain flares have been going crazy all over the board. One day nauseated from groin pain limping out of work and the very next day I have little to no pain. I called the Uro to see if I could get in before my May appt awhile back for another nerve block but no earlier times available. My May appt date is just days away.

I have been doing the research on denervation of the spermatic cord or doing an orchiectomy. Going to try the route of I have had my vasectomy and my dad days have come and gone so let’s just remove it. See how that goes thru.

No matter which way it goes on the 5th I have decided I need to end this pain. Hopefully either route that I and the Uro go thru will work and bring an end to this battle of 10+ years

Wishing you the best in your conversation and desire to resolve this. I have a very similar situation where the pain from my right testicle is so bad some days it keeps me from doing things. I have been threw a couple of Dr's and nerve blocks with no help. Thursday I have an appt with a new one. Have to see what he says.

My appt is on Wednesday just a day before yours. I wish you the best in your journey in finding an answer and solution to your pain. Keep trying be persistent, honest and upfront with the docs as to how the pain situation is and how it impacts your life. I’m in similar spot as you with right sided testicular pain along with femoral pain that is located on the inside of my right thigh after abdominal surgery damaged several nerve branches. Have you been involved with a pain management doctor yet?

You describe it to a T. The pain in my flank I can almost point to. I tried the blocks with a pain specialist. She said it would last 3 months. For 3 days was more like it, plus having a numb leg! That shot was like a horse shot!

I also tried nerve blocks early on in my journey into this black hole of pain. Yeah they hurt like hell when I got them right into the lower right abdomen in the area of the damaged nerve branches. At first I got some lasting relief but with each one they less and less effective. Pain doc finally said he would do no more. After many hurdles I got insurance to approve a pheriperal nerve stimulator to block the pain. It has worked great over the years but this year or so not so much for the testicular pain

Saw the Uro today and we discussed how the nerve block on the spermatic cord worked for my situation. He recommended the spermatic cord denervation surgery and discussed the risks possible benefits etc.it will involve being off work for a few weeks and no sex for 6 weeks. I told him that would be an issue. Oh which one? Ummm the sex issue and when I had my prostate laser surgery my former Uro in your practice gave me some shot to help out in that dept. The Uro I thought would be floored but all he said was oh he must have gave you a depo shot. No problem. Once surgery is schedule we will also schedule you for a shot at our pharmacy two weeks before and before your follow up appts.

I also mentioned I had done my research about this and medical journals either recommended this surgery or an orchiectomy. He stopped me right there. He said no way at this time would he do that unless there was a complication after microscopic denervation leaving only the blood vessels to the testicle intact. He does not want to get me into monitoring and adjusting hormone levels in case the other much smaller one can’t handle it. He was absolutely against a partial orchiectomy.

Got the call back for surgery scheduling for denervation the other day. Was given the option of a date upcoming or wait until Late June for the next opening in the surgery schedule. I told the lady I’m tired of the pain let’s take the end of May appt if we or should say she can work her magic. Well the other morning she called and the scheduler had worked her magic in getting things set up on a short notice. Today I went in and got a huge dose of depo due the short timeline hoping it will kick in by the time of surgery. Yup my buttocks hurts a little now but less than the testicle does at this point so I’m good with that. Tomorrow off for a pre op physical with my GP. A couple of other appts to work with before the surgery date. Fingers crossed this helps bring an end to the pain soon. If it doesn’t not sure how much longer I can keep working.
