I may have found a doctor in Atlanta Georgia that will perform voluntary orchiectomies. The website is www.ccrsplasticsurgery.com. Thought I would bring this to the attention of anyone considering the surgery. I filled out the contact form and got this reply.
Thank you for visiting Dr. Fara Movagharnia's website. www.ccrsplasticsurgery.com and for your message about an orchiectomy. You do not need to provide a letter from a therapist for this particular surgery. The only thing that is required from you is a letter/email from yourself describing your background as to why you would like this procedure. Please also state that you understand that this is an irreversible surgery. You can use this email address. The cost of the orchiectomy is $4000. This can be paid with cash, credit cards or financing. I would be happy to give you more information about this, so please give me a call at the number below whenever you are ready.

That is a good find. In my estimation the price is a little high, but no letter required! I paid just under $3k for mine.

Was that under local anesthesia?
I got my letters, was suppose to have orchiectomy with scrotal reduction last week but insurance fell through and urologist only performs surgery in a hospital so they wanted $20k. Back to square one, trying to find someone on west coast, prefer to be asleep during the procedure.

General anaesthesia is riskier. It generally requires a nurse anaesthetist to be present throughout the procedure. You might have better luck at a reasonably priced outpatient procedure with nitrous oxide (laughing gas). That won't put you under, but it should take care of any anxiety.

No I was put under. The doctor wouldn't do it with a local no matter how hard I tried to persuade him. My scrotectomy was done under a local however, it cost me $2500.
