Getting the letter took me 5 years and 3 and a half therapists. The first two weren't too supportive, so I had to change. From my experience during this time I think your case will be even more difficult. Probably no therapist and legal surgeon will support the idea of castrating a healthy young male just because he wants bigger balls. I don't want to discourage you, but just to get a realistic view.
I will take T after the surgery. Just because too low T leads to health risks. Actually, I'm already on supplemental T due to the fact my natural T level is significantly below the normal range. Let me add that your idea of pushing the T level up is not too good. When your level is above the normal range it can lead to breast growth and other risks for your health. The hormone levels must always be monitored by an endocrinologist, otherwise hormone replacement can be dangerous.
