The one thing I didn't mention in my intro

I was going to leave this out initially so I didn't mention this in my intro. In 2013 I had a failed back surgery. I needed a 3 level lumbar fusion and wound up getting a lamenectomy with discectomy. He told me that I still had enough disk left at L5 and L4-5 to last me for a few more years and that I need not worry. So he did his procedure at L 3/4 the only problem was he removed some bone around the nerve but left the meat of the herniated disk hanging there in my spinal canal. So for the following year I continued to have nerve pain in my groin, testicles, penis, and down the front of my legs to about the knee. I would get burning pain between my anus and testicles that would last sometimes a week or more. And just non stop electrical shocks through my groin and down the front of my legs. Then one night that herniated disk fragment broke loose and lodged itself between the wall of the nerve canal and the nerve in my left leg at L 3-4. I couldn't walk, push waste out of my body, and I was in extreme pain and bed ridden for 3 months. At one point throughout the ordeal I tried to get an erection and while I had feeling my penis was dead. And even if I somehow managed which I didn't, the pain would have been on the level of needing to scream. No matter what it was from peeing to coughing I would get this intense jack hammer pain in my upper left thigh that would bring me to the verge of passing out. I had to use a tens unit and a variety of means to pass feces and urine which was done laying the tub with the shower on. It was hell and it ironically cleared up in the same amount of time that I had been bed ridden, 3 months, 6 months in all. It took a year to find a doctor and go through the required tests to get a 3 level TLIF fusion.

So after all this goes by and I recover which took another year I was left with nerve damage in I guess the nerve roots that control orgasm and so forth. What happens is I get electrical shock sensations from the base of my penis and down the front of my legs. It also goes through the testicles and in the general area behind my scrotum. And during flair up the shocks go as far as my feet. This happens sometimes just before but always during and after an orgasm. Often lasting sometimes up to a minute plus. I also get those same shocks and sometimes a little burning in one of my testicles during nocturnal erections. And too many of them and you either remember waking up for each one or you just wake up feeling tired because you partially woke up several times. So for me orgasms just plain suck but the over active libido from high T keeps me indulging.

Now the drugs for this kind of problem have more side effects than they are worth. Most are for Parkinson's disease with nerve pain as a secondary use, or off label use. And since narcotics are a real problem they seem more inclined to not care if you suffer using a non narcotic with problems that you never had before. Finally I found CBD and began stuffing capsules with CBD isolate and popping one a day. Now I'm 50-75% better. When I was on the opiods I felt terrible from the pain and that stuff lowers your T, especially the synthetic versions which is what I was on for months. But now that I've been off of that for about a year my T seems to have really taken off. I would say I'm at the frenzied teenager level but at 45 and with nerve issues. Getting off now to satisfy a non stop urge is like that scene with Tom Hanks in The Green Mile where he deals with a kidney stone in the bathroom.

The wife lost every bit of her sex drive now that she's hitting menopause and has back problems of her own. So she's not into it anymore and I can handle her needs without my penis and have a better time if she is. And with the extreme sex drive and need to constantly masturbate all I'm doing is aggravating those nerves the more that I go. That flairs up other areas such as foot pain from L 5 nerve damage, etc.

So for me it's really pointless to keep the problem going since it's just a matter of two testicles.

I know what you're going through, I had all the symptoms of pudendal neuralgia due to L4 L5 issues.

The pain, the burning, the tingling. It was a year of hell. I went through 10 doctors before one figured out it was spinal. Put on tons of antibiotics, antidepressants, prostate and bladder medications. I can't believe how incompetent doctors are. On gabapentin which caused horrible memory problems.

Had microdisectomy which resolved many of my symptoms, but still have overwhelming arousal, unwanted erections, and some genital pain.

And likely need to go back for a laminectomy or fusion, since I also have stenosis which seems to be impinging my sacral nerves.

While my T was in 75% percentile, and nuking it with cyproterone helped a little, ultimately the arousal is primarily from nerve impingement.

And ended up with osteoporosis from being on cyproterone for 4 years. So now I'm on Fosamax and probably going to start taking estrogen.

Check your T, but that huge sex drive can just be nerves.

I'm also dealing with foot pain, I've been told it was a neuroma by a podiatrist, but can't help think it may be spinal, have an EMG in 3 weeks.

As in my case I did a awful lot of damage to my penis, I injectioned it with 95% ethanol and also hitting it with a hammer. the damage that I inflicted to it over 18months was so bad that I ended up having my corpus cavernous calcified, to the point that it started to push against my pudendal nerve. I also had a sharp shooting pain whenever I tried to sleep on one side. The doctors did not know what I had done but were willing to try different things to ease my pain. What I was hoping for was to be rid of my penis, but the way I went about it is not for anyone else to try.

bottom line. To ease the pain they first put me on a medication called CYMBALTA, used as a powerful antidepressent, take it and you forget about pain and everything else. You can become suicidal. I took it for awhile but the aftereffects were ruining my life.

Next thing that they tried were 10 sessions of xray, to sap the nerve. It did work, and gave me relief from the pain for over 2 months. Nerves do grow back, and when they do grow back the pain returns.

in the end since I knew what had caused all my problems, i requested them to remove my penis so that it was no longor pushing on the prudental nerve. That was over 5 years ago and since then my pain problems have gone away.

I know that you problem was not self induced like mine. Mine was to have my penis removed, which is what I got.

however the different treatments that were used to treat me, might be of help to you.

I've been told by Doctors cutting things off would not solve the problem, since it originates in the spine, and would be left with phantom pain.

But if another spine surgery doesn't solve this, then I'd probably explore diagnostic injection of lidocaine into the dorsal nerve to see if that provides relief before deciding what to do next.

Right now I can't lie down without feeling like I'm orgasming and need to take lunesta 3mg every night to sleep.

I did self harm from around 14 to 23 with anything I could find in the house, I'm now 38, but not sure thats contributing to what I'm experiencing now.
