I don't have prostate cancer worries. Actually It helped me get insurance to pay for my castration. I had been planning of being castrated and had my two letters from psych doctors which may have helped. But then my urologist did a prostate biopsy and found some cancer cells. It was actually, the way he explained, mild and just needed to be watched and another biopsy in two years. At my age I would die of something else before it would kill me. Anyway, the other doctor considering doing my castration said this actually would help with my insurance as they would for sure would pay for the castration since I had prostate cancer. The sad news was they wouldn't pay for my scrotum or penis removal since those would not be "medically necessary". But I am happy to be finally castrated and my scrotum and penis are shrinking to almost nothing without "T".

My PSA was starting to go up past 30. I had an indeterminant result. I started my own alcohol treatment. the second biopsy was very positive. I was given two options surgery to remove the prostate, or some form of radiation therapy. I was told to lose a significant amount of weight. or they wouldn't operate; I had already ruled out radiation; little gold-encased radioactive seeds to kill the prostate would have killed my job at the nuclear power plant. I would set off every body scanner within 40 ft. I was also told if I didn't get the surgery, it would kill me; no waiting for old age. My father had PC. The surgery was 100% effective. After nine years, my PSA is undetectable. I can now take TrT. but why. I haven't had a hint of an erection in 9 years, and with dead testicles, I don't miss it.

The most males do not understand, that’s a great risk, especially if you are older, that cancer is very dangerous. Of course it’s not so easy to understand the situation. But better to minimize the risks, than doing nothing. If there are cancer cells found, that’s time to so some against. The removal if penis is not really necessary. I am not for. Important, that the prostate should be removed. In my family the result – it was too late and the uncle died. I think it’s very important to minimize the risks.

Analyzing my 2 options going forward with my stage 4 prostate cancer.

1) Surgical Castration

2) Medical Castration (again)

With several reports it states that surgical has less side effects heart, lung, bone, etc and is overall probably less expensive. I had completed a round of medical castration that lasted 14 months (hot flashes, bone depletion, breast enlargement, higher blood pressure, Sugar) it seems every possible thing happen while I was attempting to eliminate the cancer cells was not a great experience also lowered cognitive ability and engery. Now 11 years later with a little higher blood pressure etc I am faced with the same issue. Currently I am on 2 packets of T everyday which keeps my T at around 1000 (High T replacement therapy) to keep from having castration-resistant prostate cancer , after 5 years into post Radical Prostectomy I had a pump put in so erections are not an issue any longer. Depending on the number of re-occurrence of the cancer I am leaning on the Surgical side for cost and control verse the injections/depot every month @$2K each and other side effects.

So my question is those that have faced this and chose surgical castration I would like to know after the surgery if during the times you were to have T in your system you were able to control it without having the testicles and using T gels?

Would you choose the same course again?

(Such as I spoke to several that stated they would NEVER do radiation at all again, so I have eliminated that as a therapy.)

The good part is that I am not defined by having or not having my testicles so I am looking for out comes and experiences so I know what to expect if I use the surgical aspect long term. Whereas, if it is medical it may 6-9months castrated then off depending on when it would return sometimes never sometimes within 1 year after but I know the problems with medical castration. So some unknowns.
