I think it is not necessary to get castrated. To be sure not to get PC. Then it would be better to get rid of the prostate. Because then it is not likely anymore to get PC. And when the psa levels are going to rise you can always go for an orchiectomy. Or get radiation. Then there is also no reason to start with TRT. And when you get a radical prostaectomy it is easier to operate nerve sparing. So your spouse will also be happy. And there is no cleaning afterwards.

The shock from my urologist at my suggestion I just have prostatectomy now before the very high likelyhood of the start of me getting cancer was rather annoying. I asked if I was a women considering an hysterectomy we she be vehemently against it.

Great discussion. I m glad other men are thinking about this. Lots of men do face a high risk family history of lethal prostate cancers in their male relatives. Lots of men face getting castrated at some point in their lives for this disease. So the question is, when is it most beneficial to get that castration ?

It seems that it isn't known. But chemical castration would ordinarily be done at the time of radiation treatment, for a limited duration (it isn't necessary with surgery). Then, if the cancer has spread or recurs, chemical or physical castration would be used to slow the spread of the disease.

My friends husband was just surgically castrated after discovering his prostate cancer had metastasized. He was given the option of chemical or surgical and they chose surgical.

Right choice. It's a better treatment with fewer side effects.

My family has a high risk of colon cancer since there are three generations in my family that have died from it at an early age. My doctors have brought up being preemptive in removing my colon since my chances according to the genes is 100%. I've chosen the route of frequent colonoscopies before I go that route. Why cant a male with a family history of PC go the same route?

That is the really sad part of what goes on now vs. how PC was handled in the past. It's a difficult decision for a man to accept surgical castration when offered the "same result" with medications. The sad part is that surgical castration is much more effective, fewer side effects and MUCH less money than the drugs. Particularly the newer ones that cost thousands of dollars per month.

Chemical castration from the medical standpoint of curbing PC, is nothing more than a money making program bankrupting patients with false hopes of regaining their "masculinity" while the medical profession particularly the drug companies, make a ton of money pushing this option.

AND THERE YOU HIT THE NAIL ON THE HEAD the drug companies raking in the cash by keeping men intact. I love that we actually see what is going on. I hate that we can't seem to change it. There simply aren't enough of us.

When she told me about her husband. She basically said the conversation was biased toward chemical. She asked how long he would be on them and how much they would cost. She said the answer was indefinitely and probably not too much with their current insurance. The surgical was 100% covered by their insurance. She said he was already feeling sick from the chemo, so they chose surgical and it was done that week. He has had the hot flashes, but nothing too bad, and is doing well.
