I do not know who the doctor is, but he's on most websites where he talks about this using the name Split6. Look around and reach out to him.

For what it's worth, the practice of Brownstein & Crane has a longstanding good reputation in the transgender community. If I were looking to have my genitals rearranged, I'd definitely choose one of their docs over Arnkoff. Arnkoff sounds nice, but it also sounds like he may not be the most skilled or up to date.

No credit cards. Cash would be okay but I didn't want to be carrying around that much cash while traveling to Michigan so I asked and he took a Certified Bank Check.
