Some of the men into banding play can end up with damaged balls that go numb or atrophy and shrink down to small dead grapes in size. Read up on this website about banding and elastrators to see the different results menngot doing that. The damage seems to accumulate over time until their testicles stop working or die. Numbness would help if your pain is in the testicles or near the testicles on your spermatic cords below the banding or castration cut off along your cords.
Also, some men get terribly painful testicles after vasectomies that are called chronic testicular pain. You could search and read those websites for vasectomy related chronic pain syndrome to see what men try and how well it works for that kind of testicle pain.
Its hard to believe no one will agree to castrste you for chronic pain.maybe get a vasectomy first, to end any fertility issues that drs might have about castrating a man at your age. Your insurance will enthusiastically pay for your vasectomy at your wish to save all the insurance money any impregnations by you would cost them in pre natal and maternity costs of fathering a baby. Once you are sterilized by your own choice, drs can t refuse to castrate you on fertility issues. Plus after a vasectomy, maybe you testicle pains would be treated like chronic pain syndrome as a result of vasectomy. Castrations are done to treat that recognised medical problem . Drs might view castration for that reason as suddenly being medically necessary and agree to it.
Also, if you think your pain might be coming from your testicle cords instead of the balls, maybe a cooperative dr would agree to do some extra cutting and nut cord removals when he does your vasectomy. He could choose to do the older method of cutting open the scrotum to do your vasectomy and look around and trim off some additional organs and tubes in your scrotum to see if that would reduce your pain. Its worth looking into and seeing if a dr would be more willing if its part of a vasectomy that your insurance will definitely cover for you.

Sorry I haven't posted an update in a while, the Injections seemed to work, I got refered to a new urologest and was able to get the Left testicle removed.
This has not resolved all the pain, but has lessened it on the left side. I've had about 30% less time in pain on that side. I have not had any phantom pain. I have pain in the pelvic area. I'm able to ejaculate without pain on the left side. It's not a completely fixed situation, but it's an improvement.
I'm in the process of getting the right testicle removed. the hope is I will get the same results as the left side. Being in less pain is better than the status quo as it were.
This has not resolved all the pain, but has lessened it on the left side. I've had about 30% less time in pain on that side. I have not had any phantom pain. I have pain in the pelvic area. So I got my right testicle removed on the 19th. I'm now a neutered male.
I'm not sure of all the lingo but Neutered really works for me and still male. Wasn't looking to change my gender identity and stuff with this.
I'm working on T replacement with my Dr. not sure what method, but looking at Testopel implants or injections.
I'm still kinda thinking about prosthedics, but It feels like it's adding more cmplication to the process. I'm not having any testicular pain now. so that part is resolved. I'm still having pelvic pain, not sure if it will quiet down, but it probably will as the surgery site heals.
I do got some questions. Does the scrotum shrink pretty fast? Does any one have information or expereances about that I can read up on?

Other than the missing lumps, my scrotum didn't shrink much. After surgery it was swollen to about the same size it was before, and then it deflated into an empty sack as early as 60 hours afterwards. I feel like it got a bit 'tighter' and closer to my body after 3 months or so, but not much shrinkage. My sack was already on the small-medium size though.

I was told, like many here, that my empty scrotum would shrink like magic. It got marginally smaller, but in the end I had it surgically taken for a smooth under. So, getting it removed may well be necessary if it is important to you to have it gone.

What you describe sounds a lot like transient testicular torsion and is not that uncommon. It is most often caused by a congenital defect known as "Bell Clapper Deformity" wherein the testicles "float" freely within the scrotum much like the clanger in a bell. This leads to either a complete 360 degree twisting, which is excruciatingly painful, or transient twisting where the cords will not fully twist 360 degrees but do so enough to cause less severe pain. However, over many years the occasional pain becomes lessened as nerves are also effected by ischemia and become less sensitive. Bell Clapper Deformity is fairly easy to diagnose by exciting the cremaster muscle in the upper leg and any GP worth his salt would do this test. If caught before irreversible damage is done the condition is remedied surgically by tacking each testicle in place to keep them from twisting. Untreated BCD eventually leads to lack of blood flow causing the testicles to die, often over a period of years.
I know this from personal experience, where BCD went un-diagnosed long enough that I ended up loosing both testicles.

I was told the same after my orchiectomy, but in almost two years the damn thing seemed to hang even lower. It always managed to find it's way further and further between my legs regardless of underwear choices and was a constant source of irritation, especially in the summer months.
Finally I had the scrotum removed and it's been great since it finally healed. The smooth look is a bonus as that floppy skin was not that attractive.

Was it easy to get a urologest to remove the scrotum, or did you have to see a plastic surgon? was it covered under inshurance or was it kinda spendy?

Well I've already had the testicles and spermadic cords removed. so.... if I had that, it's not a problem any more.
