As if this is not confusing enough for the average Joe, it depends on what the test is measuring.
Here in the U.S. my doctor always orders and tests for total testosterone and free testosterone. The labs I've gone to measure the total testosterone using ng/dl, and the free testosterone using the pg/ml. It must also be noted, as I was told, that different labs may use different numerical ranges for any given test result, so there exists a variety of ranges for the same test, so a different range can exist with the same result--low or very low testosterone.
In my case, my doctor concluded that there exists a consistant correlation between the total testosterone and free testosterone indicators, in my pre and post injection and post surgery removal of my one testicle. Both total and free went from nid-range normal pre-injection, to below minimum of normal range post-injection pre-surgery, and vastly below minimum after surgery.
What I found most interesting is that despite below minimum range, my doctor indicated that both the lab and ultrasound tests did not warrant removal of both testicles at the time of the first testicle removal surgery. That conclusion told me one or both of two things; there truly is reluctance by doctors to remove any body tissue or organ if there is any indicator the tissue or organ can ve rehabilitated, or that insurance coverage creates a motivation for more than one surgery. Regardless, the first conclusion affirms that which is so often repeated on here that doctors are trained and instilled with the motto do no harm or greater harm.

There are two (sorry three) scales of measuring testosterone levels that I know of to keep it simple nmol/l or ng/ml and ng/dl the latter I believe to be used more commonly and the nmol/l scale used by some labs in Europe. I have had blood work done by two labs in the UK and its been in the nmol/l scale.
ranges for the nmol/l scale usually in the 1 to 40 range.
