It's a joke and an "easter egg" from the graphic novels (comic books)...

In the previous episode, the AllFather made a comment about Starr's head scar.

In the comic, Starr has a fight with Jesse Custer and Jesse smashes Herr Stark's head in an elevator and when he recovers, the scar resembles a the head of a penis.

That's not to be confused with the star-shaped scar around Starr's eye. That was from a childhood fight with bullies.
