Friends -

I was castrated in 2014 by my urologist. He excised my testicles through two horizontal incisions in my scrotum under local anesthesia. I watched most of the operation and saw that he divided each cord, clamped each half, and cut the two parts of each cord right where they connected to the testicles, leaving most of the cords. I can still feel the cord nubs in my scrotum 14 months later.

All was fine until a couple of weeks ago when I started experiencing pain in the cords, worse on the right side but apparent in both. I had some pain in my testicles before I was castrated. My doctor eventually removed them due to my complaints of pain. Most of the pain was self-inflicted through injections or other attempts to kill my testicles, which obviously went away after my castration. The new pain in the cords feels much like what I would feel the day after an alcohol injection - a dull throb. In many ways it feels like my testicles have grown back, which I know is ridiculous.

Has anyone experienced pain in remnants of their spermatic cords months after castration? I'm not sure if he'll do it, but I'm thinking about asking my urologist to now remove the cords, too. Does anyone know if Dr. Arnkoff would?

I also have some occasional pain in my cords, also more on the right side than the left, but that could be because the urologist who did my surgery had to cut the left cord higher than the right due to a large spermatic granuloma in that cord. I was done on March 16th 2013. From the feel of it, I would say that my right cord was severed in much the same way as yours, but I was asleep during the procedure which was done through a single mid line incision on my scrotum. The feeling in the cords is akin to a light case of 'Lover's Balls" like we all used to get as teenagers.

In one of my follow up visits, my urologist warned me that further down the track I may experience this pain and if so to come back and see him and he would remove the cords inguinally. I want to have this done in the near future as I also want to have my empty scrotum removed and it would probably be wise to have the cords removed totally rather than to be pushed up into the canals during the scrotum removal.

I would suggest that you go back to the same urologist who originally did your surgery and talk to him/her and see what they say. My urologist's advice about possible future pain and removal makes me think that getting the cords removed would not be a problem, since the testicles are already gone, there can't really be an objection to finishing off the process and there is also a strong possibility you may be able to get this done through your insurance, whereas with Dr Arnkoff, you would be up for quite a hefty fee as he would pretty much charge more for inguinal removal of the cords than a normal castration as I very much doubt he would be prepared to do this in his rooms and also whether it could be done under local.

Hope you manage to get yours removed soon as I am looking forward to getting rid of my cords too.

That's also my question, how to operate with the cords. I had hernia operation, so only scrotal removal ist possible, without facing complications. How much of the cords should be removed, that they will not give pain in future? And how should it be prepared, if later a scrotum remove is wanted?

I didn't have any cord pain after Dr. cut off my balls and nutsac in 2013. I think he removed as much of my cords as he could and the little nubs that were left shrank up and crawled up inside my gut after about a month or so. Never had any problems.

Yes,

Depending on the method. I have a little bit of pain over the years

I was concerned about the possibility of nerve blooming after surgery or odd adhesions but so far no worries about that.

I think one day I may also have to had the inguinal region on both sides opened again to have the cords totally removed as well as the cremaster muscles, all of which are now palpable in the half a scream I have now.
