Has Anyone Used Dr. Movagharnia

Has anyone used Dr. Fara Movagharnia in Atlanta for an orchiectomy? From what I read on his website, he only requires a letter from the patient convincing him the patient actually desires castration and is aware of the consequences. His website gives his office phone number and says call if you want more information including prices so I think I'm going to call them.
