Yes, having been through the adventure of a urologists effort to remove a scrotum, much still remains and is a job for a plastic surgeon to make flat and tidy. Maybe there are thoughts here on how to find a good US scrotum remover?

He (Dr. Fara) is an excellent plastic surgeon and does voluntary orchiectomies without a shrink too. He did mine and after 6 months you couldn't even see a scar.
