32 hours later the cord is now aching right up the inguinal canal area it is very tender to touch.

It is not unbearable and I don't plan on poking too much.

Remarkable that it has taken this long for the pain to ramp up. This is completely unlike a testicle injection and seems to spread out....

The epididymus is also swelling and so is the testicle.

It looks like first injection somehow clogged / sealed / damaged your Vas Deferens, the second injection by gravity force took longer to pass through damaged part of Vas Deferens and reach your testicle

Overnight the testicle and epididymus have continued to swell, no great pain. Clearly the liquid will flow back into the testicle by gravity.

swollen to the size of a 'normal' testicle now.

So inflamed cords = inflamed testicle. Interesting.

The testicle remains very large as does the cord. Minimum discomfort, swelling seems to be staying longer than used to.

Testicle remains very large, much longer than previously. The swelling feels different and I think it is inflamation not coming from the liquid. The cord is still very thick going riight up into the abdomen and is a bit uncomfortable.

Inflammation normally recruits the immune system to act. You can expect macrophages to have infiltrated the tissue, and I bet the blood/testis barrier has been seriously breached for the first time in your life. Or, possibly, the second. Usually, the second time you're exposed to a new antigen, the immune response is much more vigorous, and you can see this elsewhere in medicine - placenta previa, vaccinations requiring a booster shot, you not getting sick the second time you're exposed to measles, & etc.

I'd expect you're having a modest autoimmune reaction now, and I bet you'll have circulating anti-sperm antibodies within a month detectable by immunofluorescense assay. If that's the case, this time you may have done quite a bit of damage at the microscopic level.

As I believe has been already suggested before in this thread, severe swelling of the spermatic cord may either restrict or completely stop the flow of blood to its associated testicle, the collateral damage from is that even a short interuption in the blood supply to a testicle is enough to cause irreparable damage to it. Well the collateral damage is significant as the cord remains swollen right into the abdomen and presents an awkward discomfort, bending, sitting...... I live with so much pain generally, it is not more than an annoyance. The swelling in the the testicle is now going down slowly...

I daresay ZueterMe is right and I have done significant damage. Hopefully, it won't be long before I get to hospital.

I didn't even think about the possibility of significant inflammation within a membrane-bound space. That's why head trauma frequently results in removing part of the skull and stashing it in the abdomen for later use - the swelling causes pressure that damages sensitive tissues, since there's nowhere to expand.

In rats, a 5h reduction in flow to approximately 70% of the normal value may be sufficient to induce death among spermatogonia and early primary spermatocytes. High-grade ischemia causes necrotic damage to all parts of the testis.
