Man in Jakarta has penis cut off

Not clear if he did it himself or was actually attacked. In any case it couldn't be reattached.

Police in dark over penis-cutting case

The Jakarta Post, South Tangerang | Jakarta | Thu, May 16 2013, 7:11 AM

A- A A+

The police are still gathering information on the assault of Abdul Muhyi, 21, in which the attacker cut his penis off on Tuesday, an officer said on Wednesday.

"This is an aggravated assault case. We have to find the perpetrator," Pamulang police chief Comr. M. Nasir said.

The resident of Sawangan in Depok, West Java, was hospitalized at the South Tangerang General Hospital after he came to the Pamulang public health clinic early on Tuesday with his penis already cut off.

Muhamad Mursal, a surgeon at the hospital, said the patient arrived long after the safe period to affix the penis.

At first, Muhyi mentioned a woman's name, a resident of Kemuning, West Pamulang, but according to Nasir, she did not recognize the victim.

"We are collecting information from witnesses at the crime scene in the Jalur Pipa Gas area in East Pamulang subdistrict," Nasir said.

The area is known as hangout for transvestite sex workers.

What I find amazing is not so much the number of amateur penectomies (whether self-inflicted or done during an assault) but rather the near 100% survival rate. I know some of the victims have been in serious condition when they get to the hospital, but very few seem to die from blood loss. I would have expected a higher mortality rate. Could do-it-yourself surgery be safer than we think? (Not that I'm recommending it, mind you.)

They had prompt medical attention afterwards, if it is not provided, blood loss would be quite fast.

Without medical help one has three potential problems:

1. In short time frame - shock and blood loss.

2. In medium time frame - infection.

3. Long term - urethra healing shut.

4. Price (as medical help would be needed anyway), and potential problems if this is qualified as self harm.

I think one mitigating factor is the flaccid/erection aspect of a penis. An erect penis has an incredible amount of blood pressure, i know that the few times I've punctured it the blood shot across the room in huge spurts like in a violent movie, every pump of the heart was like splurt, splurt, splurt. But a flaccid penis has almost no blood flow and in fact the tissue of the penis can survive longest of any of your flesh with low oxygen condition (which is why banding a penis isn't as damaging as banding a finger for example). While I think some guys might be erect when cutting the penis, I think the brain probably turns that to flaccid really fast when you're actually cut.

Another factor is that it is very easy to tourniquette or put pressure on the penis to stem the flow of blood. I know when I have cut myself, as in the examples above where I was bleeding badly, I was able to of course just grab my penis to ensure I didn't bleed too much. Even if it was cut off down to a stump, you can basically just press against the wound to help stem the flow.

The guy in the other article I posted in England did lose a lot of blood, and was apparently in pretty bad shape when found. It was enough to make him go unconcious, which is a lot of blood loss, and they described big pools of blood in the street.
