In many jurisdictions if it seems that you self-harmed you can be put under involuntary psychiatric hold and monitoring. Plus it will go on your medical record. In the US it can complicate your insurance coverage -- for example every time I go to ER here, insurance inquires whether it was an "injury" and if you say yes then they'll try to investigate because they will want to sue the homeowner or workplace or auto insurance depending on where you claim the injury happened.
I think it is much better to see regular doctor with drastically atrophied testicles than to go to ER with obviously traumatized testicles.
Like jcat, I used a very small needle and there was no visible trauma or bruising, plus I had a good cover story which fitted with my medical records which were on line here in Aus. About 3 months prior to my visit to the ER, I had seen my doc about a massive intra scrotal infection which I told them had possibly recurred. I also took the scans from that infection with me. There were no questions asked or eyebrows raised. I guess the major thing in situations like this are to have a history going back a while as well as a really good cover story which fits in with the medical records.
I do not advocate anyone else following this path, but it worked for me.
