Diabetes, smoking, chronic alcoholism

Scrotal inflation or infusion

Any non-sterile invasive procedure. (pins and needles or cutting in genital area)

Very rare but docuccmented: excessive masturbation with lubricants, even soap.

Lack of sanitation

Or just plain old bad luck.
