Please explain, Paolo. Are you saying medical care will improve if doctors and patients lose control, and unseen non-medical bureaucrats decide who gets what care?

Are you saying existing HMOs which also seem to do the same thing, but are run entirely by private enterprise, have improved medical care?

Lets see, I remember needing disc removal surgery years ago, the first thing was an x-ray where the doctor said my next step was to call my insurance carrier so they could decide my next move, that they would decide what doctor I went to and hospital I stayed at, its was all up to the insurance company, the doctors had no say in this. Several years later while in the hospital my doctor demanded I stay one more day but the insurance company said that was it, no more. I should have stayed at least one or two more days as I was still very sick but the insurance companies know best after all. Then there was the time I was denied a claim because I did not get prior approval for the rescue unit to take me to the hospital, of course I was out cold and could not even tell you my name, but after all the Insurance companies knows best, after all its about the profit, you know they don't do this for free, they must make their 40% profit after all, corporations being people and everything.

Yes Bob we do agree on that, that doctors should be the ones calling the shots, not insurance companies or the government.
Having said that I do believe that we should have national health care, where everybody pays into the pot from cradle to grave, that it should be run like any number of other counties as there are many to chose from. I believe our drugs should be up for bid and not pushed on us by drug companies which they seem to do at each commercial break, followed by the law office of, who is going to get you the money you deserve because the medical treatment you received might not have worked. Bob both practices should be eliminated and that alone would cut our costs for medical care in half. But I do think we agree that the wrong people are calling the shots, it should be left up to the doctors. Having said that, I also think dental and eye care should be included in that basic package.

And Reputation Tag Too.
Now if we can just restructure physician payment away from procedures and more towards "time".
Primary Care physicians lack procedures so unless they are employees they cannot survive.
Especially with the HillaryCare requirement of being in a "network".
There is a spiritual side of medicine, beyond the check lists that plays a role in the diagnosis and recovery of a patient. The Free Market would allow those who are "comfortable" with one another, seek each other out.
Insurance should be able to manage it.
Computers should be able to ID health care charges that are out of the norm from one entity.
It seems so simple, self evident. Why not?

My view of doctors is the same as William Shakespeare's view of lawyers - kill them all.
I lost ALL respect for, and faith in, the medical community when I found out the hard way that I was a Type-2 diabetic.
"Their" idea of how to treat it is nothing more than a hamster running a treadmill - carb up and shoot up and pay pay pay.
My way of treating it costs me about $20/month, in control strips.
I eat low-carb and mostly natural foods. High fat, high protein.
Tell this to any doctor and they'll be shocked and appalled.
But it works.
I have spent "0" on pills and/or insulin, and I don't need it.

It all depends on the doctor's knowledge and willingness to learn. My doctor would agree with Paolo's prescription, other than adding a couple of glasses of wine to dinner to keep the liver busy and not turning carb into blood sugar. He's read the European literature that has tested that route.

When the <doom> Federals <Darth Vader breath sounds> are issuing missives to treat Diabetes with good complex carbohydrates such as brown bread, pasta, Special K and non fat milk - and avoid the evils of animals fats, but margarine is okay 
It is really, really hard, time consuming and dangerous for the doctor to teach good, Paleolithic eating habits.
Explaining it was old school that rough difficult to digest food aid the excretion of cholesterol. No theory.
I know. I did it anyways. Reduced some by 2 BP meds and DM meds, step by step too over 2 months.
It takes lots of unimaginable time to deprogram someone, then to teach them. And of course should they have had an incident such as a stroke or MI, liability kicks in. Nothing just happens.
Then get treated to "anger" yes "anger" - why was I the first to teach them, justify myself. <sigh>
One of my fav profs was one of the longest living insulin users in the seventies. One of the first to receive the hard to manufacture, brown, "what is a unit?" - insulin.
It's the carbs and it is the Nanny State's fault! :realpisse
