I've just wrote Dr. Arnkoff a letter this afternoon. Told him all I could think of might be useful for him to know about myself, that I want my testicles and scrotum removed and asked him if he'd require a letter and so on. I just have to wait now.

When I talked to Dr A about having my scrotum removed he said to wait at least 6 months before its removal.

He said it was because of the swelling and by that time the scrotum should start to shrink.

I think it is a good idea to wait at least 6 months to have the scrotum cut off. That is what I did and like the result. There are different things to deal with in each case so better not to mix them together. After the castration process is totally healed...at least three months, then you can see what is the skin situation is with the scrotum. By 6 months the scrotum removal will be easy to define and any issues with it will have nothing to do with the castration. It is easier then for the surgeon to mark out the incision line to get exactly the result you want. In my opinion there is little to no shrinkage of the empty scrotum. I detected none after 10 months. Hope that helps.

That is some great info, i don't want any scrotum left and was debating between Arnkoff and Fara, since Fara could do both. Since it seems best to separate the two procedures, I will go ahead with Dr. A. I suppose any plastic surgeon would be able to remove the scrotum at a later date.

After having a conversation with the doctor, I scheduled an appointment for the 24th of September. My nex question is hormones or not?

I think you need to explain more about what you want to achieve. At the very least you should take medication for osteoporosis and that might be hormonal. If you want to feminize you should take estrogen. If you want to just do enough to avoid side effects of testosterone withdrawal then you'd take a bit of testosterone. If you wanted to return to male functionality then you'd take full does of testosterone.

The other aspect of the decision is cost. Getting HRT isn't necessarily covered by insurances, and if it is you might not get much choice in terms of what type. If you don't have insurance then it can add up.

Lastly it depends on the application. Do you really want to be self-administering injections every week? I've done a lot of steroids, but never really got used to sticking a needle into myself.

I spoke with Dr. Arnkoff for about 10 min. the following sunday that I sent him an email, after telling him some infos about me, age, weight, medication and so on, he said he would do it, then I asked him about the letter he mention on is web site, that he may ask or may not ask, his answer was "Naahh". Perhaps he'd have given a different answer if I would have said I was 20 y.o. He took time to explain to me how to

prepare for the surgery, what to do, to stop some medication before surgery and finally he said that he'd email me all infos that I'd need for the surgery. He said he can book me an hotel room nearby, He recommends to stay in the city over night for a check up the day after. I felt comfortable talking with him. I'd trust him

He used to do SRS for transexual, he seems to have reoriented his career perhaps for less stressful work. At no time he has questioned me about my desire as if it was a normal request.

Like paring, I spoke with the doctor and received much the same response. As soon as I can coordinate with a friend to go with me, I will be making my appointment.
