You're right. I just loaded both pictures and kept switching between them, and it's easy to see that he's in the exact same position. The white block that his penis is surrounded by is also inexplicably fragmented in the "after" picture. So it's most probably a fake.

I wonder if anyone's done this for real... I'm suddenly reminded of a video I saw where a guy was tied up and blindfolded, and another guy cut off his penis with what looked like a huge pair of wire clippers (he held one handle in each hand).
