Since writing the question I read the IRS pub on the subject of medical expense deductibility.
I just read the IRS Pub and it seems to confirm that it is a deductible item, including lodging and transport. The only caveat is that only those costs above 7.5% of your AGI are deductible.

Hi everyone,
Getting ready to file for last year, the year I was castrated. I was wondering if anyone would offer their opinion about if the expense is deductible. Although I knew it was best for my mental health, and it was, I didn't receive an Rx for the procedure.

Dr Kimmel castrated me and removed my scrotum at te same time, 17 Nov 03. I don't know if I can comment on the two surgeries separately, but I like Dr Kimmel as he honestly seems concerned.

I have to aree with DeepCore; my surgery was about 1.5 hrs with Dr. Kimmel (Spector associate) 4 months ago (17 Nov. '04). I can't say how many shots I received, but by suture time, after balls and sac removal, the only thing I felt was the outer reaches of the sac area, but minor.

I was castrated and had my scrotum removed on 17 Nov. 2003. Dr Kimmel, Dr Spector's associate who did the procedure in Philadelphia, PA, was very thoughtful and considerate and he responded positively when I requested pain meds for probably a month afterward.
